Case Management Services

The Case Management Program provides short-term counseling and linkage to long-term assistance to people in need. Through this service clients can speak with a licensed social worker who can assess problems and identify resources.

• Case Management programs are limited to residents of Franklin and surrounding counties.
• Proof of being under a physician's care is required.

Depression Screenings and Counseling

Having diabetes may be overwhelming at times. Central Ohio Diabetes Association offers depression screenings and counseling by a licensed social worker.
