In order to register online, participants must make payment during the same time they fill out the registration form.  

Those forms that are received without a payment will be considered invalid.  All payments received after October 8 must also include a $25 late fee. 

If three or more professionals attend from one organization, deduct $5 from the one-day fee, or $10 from the two-day fee, for each attendee.  All of the GROUP DISCOUNTED registrations MUST be mailed in one envelope.  If your employer is sponsoring you, your completed registration form must be included with the payment. 

The Central Ohio Diabetes Association annual Diabetes Symposium is a conference for health professionals seeking continuing education hours in diabetes.  Nurses and nurse practitioners, certified diabetes educators, pharmacists, and dietitians obtain CE or contact hours while increasing their diabetes knowledge, skill level, and acumen.  Other health professionals are welcome to register, attend and receive a certificate of attendance, but will not receive continuing education credits.
